On April 3, 2022, GA MedGroup became aware of a disclosure of Protected Health Information (PHI) of some patients.
This disclosure was the result of an appointment book being stolen from a locked vehicle. The stolen information included full patient name and the name of the facility where services were provided. No other information such as Social Security Number, financial information, or clinical data was included. The investigation of this incident is complete at this time and no action is required on your part.
We are committed to maintaining the privacy and security of health information and have taken the following steps in response to this disclosure:
- An investigation has been conducted to identify process improvements.
- Guidelines have been reviewed to help prevent similar disclosures.
- Appropriate referrals have been made to authorities for additional review.
